HomeFirst

HomeFirst is a rapid response and case management service which helps people stay well and independent. The service supports the frail elderly and people with multiple long term or complex conditions to remain at home rather than going into hospital or residential care.

HomeFirst brings together community health, social care, mental health and voluntary sector agencies within Hertfordshire to provide the right treatment at the right time in the right place with the ultimate aim of reducing accident and emergency attendances, unplanned hospital admissions or placement into residential care.

The scheme is currently operating in in the Lower Lee Valley area (Cheshunt and Broxbourne) and North Hertfordshire (Hitchin/Letchworth/Baldock), with plans to roll out the service across the whole of East and North Hertfordshire by the end of 2016.

Community Midwife

The midwife’s role at this surgery is to monitor the wellbeing of mothers and babies during the antenatal period, following the NICE guidelines on antenatal patterns of care.

The midwife holds an antenatal clinic, by appointment, every Monday. Please enquire at reception for details.

Further antenatal clinics are held at the Drop-in Clinic at Waltham Cross Health Clinic on Monday and Friday from 9.30 – 11.00am. There are also sessions held at Hoddesdon Health Centre on Tuesday and Thursday between 9.30 – 11.00am. Evening clinics are also available at Hoddeson and Waltham Cross Health Centres by appointment.

The midwife aims to maintain continuity of care postnatally, when home visits will be undertaken to advise, supervise and support new mothers.

Alison Johnson supports the practice in helping families to make healthy lifestyle choices.
