Before filling out a registration form, check you are in our catchment area.
Registering with the surgery
To register with the surgery:
- fill out an adult new patient registration form online if you are 16 years old or over
- fill out a child new patient registration form online if you are under 16 years old
As the practice is an accredited safe surgery, no documentation is needed to register if you live within our catchment area.
The practice is not part of the out of area registration scheme therefore patients moving out of our catchment area will be asked to register with a new GP nearer their home address.
Please allow up to 7 working days for us to process your registration before contacting the surgery
When you register, it’s important to have your NHS number. You can use the NHS website to find your NHS number.
